The CMS 5-star rating system is a nationally recognized benchmark for assessing the quality of skilled nursing facilities. It is designed to help potential residents, and their families, easily compare communities and make informed decisions about nursing home placements. The overall rating is based on a comprehensive evaluation of three primary areas: health inspections, associate staffing, and quality measures. After extensive research, on-site visits, and data analysis, each facility is assigned an overall rating between one and five stars. A five-star rating indicates a facility is delivering care that is considered "much above average" compared to others across the country.

Health inspection ratings are derived from standard inspections and complaint investigations conducted over the previous three years. The associate staffing rating measures the number of care hours provided daily per resident by registered nurses and other nursing associates, while the quality measures rating is based on 16 different clinical and physical indicators that reflect resident well-being, such as mobility, pain management, and activities of daily living.
